Job Summary
Working remotely in a full-time capacity, the New York Licensed Clinical Coordinator will manage member engagement in the Long-Term Support Services (LTSS) Program, ensuring access to resources and facilitating communication between customers, health plans, and community organizations.
Key responsibilities:
- Build lasting relationships with members, families, and caregivers
- Conduct follow-up calls and ongoing assessments for new members in the LTSS program
- Promote understanding of health delivery systems and assist customers in addressing unmet needs
Required qualifications:
- Bachelor's Degree in a Health or Human Service field preferred, or equivalent experience in managed care or health services
- Working knowledge of Medicaid programs and extensive experience with LTSS benefits
- Experience in Managed Care or with Social Service Agencies
- Ability to incorporate Evidence Based Innovation practices into customer engagement
- Understanding of New York State Medicaid regulatory guidelines and person-centered planning methodologies
