Job Summary

A company is looking for a Therapy Management Clinical Pharmacist (NY-Licensed).

Key Responsibilities

• Review and evaluate medication orders for accuracy, appropriateness, and safety
• Conduct patient care assessments and collaborate with healthcare providers on medication therapy plans
• Monitor and manage drug interactions, adverse effects, and therapeutic outcomes

Required Qualifications

• Doctor of Pharmacy (PharmD) degree and licensed pharmacist in New York
• Strong clinical knowledge relevant to the Therapy Management program, particularly in diabetes mellitus and/or weight management
• Experience in a healthcare or hospital setting
• Commitment to continuing education and professional development
