New York Licensed Clinical Pharmacy Manager

Job Summary

Supporting a centralized clinical pharmacy model, the full-time New York Licensed Clinical Pharmacy Manager will manage remote medication order verification, clinical consultation, and tele-pharmacy services across various care environments, enhancing medication safety and care transitions.

Key responsibilities

• Perform centralized remote medication order verification and collaborate with healthcare teams to resolve medication-related issues
• Support Hospital-at-Home initiatives by reconciling and managing medication orders, including telepharmacy consultations
• Conduct medication reconciliation during transitions of care and provide virtual counseling for discharged patients

Required qualifications

• Doctor of Pharmacy (PharmD) or BS Pharmacy
• Active pharmacist license in New York
• 2-5 years of experience in hospital or health-system pharmacy
• Experience with remote order verification and telepharmacy
• PGY1 residency preferred; PGY2 residency in ambulatory care or clinical pharmacy is a plus
