Job Summary

Seeking a New York Licensed Health Home Supervisor, the full-time position will oversee a team of care managers to ensure effective delivery of Health Home services, focusing on staff supervision, care coordination oversight, and quality assurance in a remote setting.

Key Responsibilities

• Supervise and support Health Home Care Managers and care coordination staff
• Monitor staff caseloads, productivity, and documentation for compliance with program requirements
• Conduct regular case reviews and provide coaching and training to team members

Required Qualifications

• Master's degree in Social Work, Nursing, Public Health, Healthcare Administration, or a related field
• 2-3 years of experience in care management, case management, or healthcare coordination
• 1-2 years of supervisory or leadership experience preferred
• Knowledge of Medicaid Health Home programs and care coordination models
• Experience with Electronic Medical Records (EMR) and care management platforms
