Job Summary

A company is looking for a Home Health RN Case Manager.

Key Responsibilities

• Provide support to dual eligible populations, addressing healthcare and social determinant needs
• Develop proactive care plans and conduct assessments to evaluate members' needs
• Collaborate with interdisciplinary team members and engage members using motivational interviewing

Required Qualifications

• RN License required
• 3-5 years of clinical experience required
• 2-3 years of case management, discharge planning, and/or home health coordination experience
• Bilingual candidates preferred
