New York Licensed LPN Coordinator

Job Summary

To support the Home Care and Hospice team, the full-time New York Licensed LPN Coordinator will manage patient referrals, coordinate service requests, and ensure efficient processing while working a hybrid schedule of remote and onsite duties.

Key responsibilities:

• Screen inquiries from referral sources and provide accurate information regarding admission criteria for home health care and hospice services
• Input admission data and coordinate service requests to ensure timely patient admissions while addressing specific care needs
• Build and maintain productive relationships with internal and external customers, offering assistance with the referral process as needed

Required qualifications:

• Graduate of an accredited Licensed Practical Nursing program with a minimum of 2 years LPN experience
• A minimum of 2 years customer service experience required
• Valid NYS LPN license
• Knowledge of insurance and reimbursement guidelines preferred
• Basic computer skills, including navigating a Windows-based environment and managing electronic documents
