New York Licensed LPN Coordinator

Job Summary

To support home care and hospice services, the full-time New York Licensed LPN Coordinator will manage patient referrals, coordinate service requests, and ensure efficient processing while working a hybrid schedule of remote and onsite hours.

Key responsibilities:

• Screen inquiries from referral sources and provide accurate information regarding admission criteria for home health care/hospice services
• Input admission data and coordinate service requests to ensure timely patient admissions
• Build and maintain relationships with internal and external customers while assisting with the referral process and special projects

Required qualifications:

• Graduate of an accredited Licensed Practical Nursing program with a minimum of 2 years LPN experience
• A minimum of 2 years customer service experience required
• Valid NYS LPN license
• Knowledge of insurance and reimbursement guidelines preferred
• Ability to learn and utilize new systems and computer applications as necessary
