Job Summary

Key Responsibilities

• Assess and evaluate the needs of complex members, collaborating with various stakeholders to identify gaps in care
• Develop and communicate medical management strategies and interventions to ensure high-quality, cost-effective care
• Coordinate interdisciplinary team tasks and activities while advocating for members and facilitating their care plans

Required Qualifications

• Bachelor's degree
• Current active RN license
• 4 - 6 years of clinical experience
• Organization and prioritization ability to manage a complex caseload
• Experience in case management/care coordination, managed care, and/or utilization management (Preferred)
