New York Licensed Nurse Case Manager

Job Summary

Working remotely full-time, the New York Licensed Nurse Case Manager will conduct telephonic and in-person assessments, develop individualized care plans, and coordinate services for members enrolled in Managed Long-Term Services and Supports (MLTSS).

Key responsibilities

• Conduct comprehensive telephonic assessments and in-person evaluations to determine medical needs
• Develop and authorize individualized care plans within MLTSS/FIDE program benefits
• Document all assessments and interactions accurately in the electronic health record

Required qualifications

• Active, unrestricted RN license in the state of New York
• Minimum of an associate degree in Nursing
• Case Management Certification (CCM) preferred
