Job Summary
Empowering families for optimal wellness, the full-time remote New York Licensed Nurse Case Manager will assess member needs, develop individualized care plans, and coordinate medical services for high-risk populations in a telephonic managed care setting.
Key responsibilities
- Assess the physical, psychological, and social needs of members and develop tailored care plans
- Monitor care plans and collaborate with healthcare providers to ensure effective service delivery
- Educate members on their conditions and treatment plans while facilitating access to community resources
Required qualifications
- Current, unrestricted Registered Nurse (RN) license in New York State
- Three or more years of clinical experience in a Level III or above NICU
- CCM certification required within the specified timeframe
- Experience in a Case Management role is preferred
- Proficiency in data entry into databases or EHR systems
