New York Licensed Nurse Evaluator
Job Summary
A company is looking for a Nurse Evaluator.
Key Responsibilities
- Evaluate new and current NHTD Waiver participants and make recommendations for level of care changes
- Conduct informational meetings and community outreach to disseminate information about waiver services
- Monitor waiver services and expenditures, review and approve service plans, and provide training and technical assistance
Required Qualifications
- Graduate of an accredited Registered Nurse program
- Current licensure as an RN in the state of New York
- Two years of satisfactory professional experience working with individuals with disabilities and/or seniors in a community setting
- Clean, valid NYS driver's license (no major violations within the last 5 years)
- BS or AS in Nursing
