Job Summary

Providing after-hours telephonic care, the part-time New York Licensed Nurse Practitioner will manage high call volumes, perform telehealth visits, and deliver medical support to aging residents across multiple states while working remotely.

Key responsibilities

• Serve as the first line of support for residents and facility staff after-hours, offering medical care and direction over the phone
• Utilize Curana's telephonic platform to coordinate care between facilities, hospitals, and clinics
• Conduct comprehensive assessments and document encounters accurately in the EMR to ensure compliance with CMS requirements

Required qualifications

• Master's Degree as a Nurse Practitioner
• Current unrestricted NP license in New York; active or willingness to obtain licensure in additional states
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Background in acute and chronic disease management, with clinical experience in adult, family, or geriatrics
