Job Summary

To support health plan partners, the part-time New York Licensed Nurse Practitioner will perform comprehensive virtual health risk assessments, focusing on whole person health and utilizing pre-populated clinical templates for efficient documentation in a remote setting.

Key responsibilities

• Conduct virtual health assessments, including medical history reviews and screenings
• Maintain a self-managed schedule with availability across days, evenings, and weekends
• Educate members on whole person wellness and connect them to appropriate resources

Required qualifications

• Active, unrestricted NP license in New York state
• Board certification (ANCC, AANP, or NCCPA)
• Current BLS certification
• Proficiency with EMR systems and comfort using digital tools
• Strong clinical assessment and diagnostic skills
