New York Licensed Nurse Practitioner

Job Summary

To support patients with advanced illnesses, the full-time New York Licensed Nurse Practitioner will conduct in-home and telehealth assessments, manage symptom treatment, and collaborate with an interdisciplinary team to enhance palliative care services remotely.

Key responsibilities

• Conduct in-home and telehealth assessments, formulate diagnoses, and deliver care aligned with patient goals
• Prescribe medications, perform medication reconciliation, and facilitate family meetings and goals of care discussions
• Educate patients and families on disease trajectories and coordinate care with the clinical team

Required qualifications

• Master's degree in a Nurse Practitioner program or equivalent post-master's certification
• Current New York State Nurse Practitioner license and registration
• National Board Certification in Advanced Practice Nursing required for all NPs hired after June 1, 2019
• 3+ years of experience in palliative care or geriatrics
• Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required
