Job Summary
To support patients with advanced illnesses, the full-time New York Licensed Nurse Practitioner will conduct in-home and telehealth assessments, manage symptom treatment, and collaborate with an interdisciplinary team to enhance palliative care services remotely.
Key responsibilities
- Conduct in-home and telehealth assessments, formulate diagnoses, and deliver care aligned with patient goals
- Prescribe medications, perform medication reconciliation, and facilitate family meetings and goals of care discussions
- Educate patients and families on disease trajectories and coordinate care with the clinical team
Required qualifications
- Master's degree in a Nurse Practitioner program or equivalent post-master's certification
- Current New York State Nurse Practitioner license and registration
- National Board Certification in Advanced Practice Nursing required for all NPs hired after June 1, 2019
- 3+ years of experience in palliative care or geriatrics
- Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required
