Job Summary
To support patients with advanced illness, the full-time New York Licensed Nurse Practitioner will conduct home and telehealth assessments, manage symptom treatment, and collaborate with an interdisciplinary team to facilitate high-quality palliative care.
Key responsibilities
- Conduct in-home and telehealth assessments, formulating diagnoses and delivering care aligned with patient goals
- Prescribe medications, perform medication reconciliation, and facilitate family meetings regarding care plans
- Educate patients and families on disease trajectories and coordinate care with healthcare providers
Required qualifications
- Master's degree in nursing or an approved supplemental education program for Nurse Practitioners
- Current New York State License as a registered nurse and certification as a Nurse Practitioner
- National Board Certification in Advanced Practice Nursing (for NPs hired after June 1, 2019)
- 3+ years of experience in palliative care or geriatrics
- DEA certificate and Basic Life Support (BLS) certification required
