Job Summary

Providing remote care to palliative care patients, the per diem New York Licensed Nurse Practitioner will manage patient needs through video visits, communicate empathetically with patients and families, and collaborate with the palliative care team to coordinate treatment plans.

Key responsibilities

• Deliver primarily remote patient care, addressing triage needs and urgent concerns via video consultations
• Educate and support patients and families, ensuring clear communication and understanding of care options
• Work with the palliative care team to assess patient needs and coordinate comprehensive care plans

Required qualifications

• Master's degree in Nursing
• New York State Nurse Practitioner Certification
• Registered Nurse license from the New York State Education Department
• Experience as a Nurse Practitioner in palliative care, hospice, or a related field
• Strong documentation skills and the ability to work independently in a fast-paced environment
