Job Summary

A company is looking for a New York Licensed Occupational Therapist (Care Navigator) to provide remote support to patients.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct users in therapeutic exercise and functional training through a technology platform
• Maintain records and provide feedback to clinicians on patient progress

Required Qualifications

• Active New York OT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms
• Experience in behavioral modification; Health Coach Certification is a plus
