Job Summary

To provide advanced-level, patient-centered care remotely, the full-time New York Licensed Palliative Care Clinician will deliver palliative-focused direct patient care, conduct telehealth visits, and collaborate with interdisciplinary teams to enhance the quality of life for individuals with serious illnesses.

Key responsibilities
  • Deliver palliative-focused direct patient care, ensuring compassionate and evidence-based practice
  • Conduct telehealth visits for symptom management and medical decision-making for members with complex needs
  • Complete comprehensive evaluations to create individualized care plans addressing physical, emotional, and psychosocial needs
Required qualifications
  • Currently licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in New York
  • 2+ years of experience in palliative care, hospice, oncology, geriatrics, or a related field
  • Clinical competency in advance care planning and serious illness conversations
  • Solid primary palliative care skills and comfort with tertiary/specialized palliative care skills
  • Proven ability to manage complex caseloads and documentation in fast-paced settings
