New York Licensed Palliative Care Nurse

Job Summary

A company is looking for a Palliative Care Nurse Navigator.

Key Responsibilities

• Conduct comprehensive symptom assessments and facilitate end-of-life care discussions with members
• Engage with members to address clinical issues and manage escalations effectively
• Collaborate with healthcare teams and support members with serious illness throughout their care journey

Required Qualifications

• Bachelor of Science Degree in Nursing
• Compact unrestricted registered nurse (RN) license; New York, Illinois, and/or Michigan licensure preferred
• At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice
• Experience caring for oncology patients and their caregivers
• Certifications such as Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications are preferred but not required
