Job Summary
To support a growing practice, the full-time salaried New York Licensed Palliative Care Nurse Navigator will conduct comprehensive symptom assessments, facilitate end-of-life care discussions, and engage with members via phone, email, and video communication while working remotely.
Key responsibilities
- Conduct conversations about goals and values, advance care planning, and palliative care for members with serious illness
- Collaborate with Nurse leaders and Medical Directors to ensure alignment with clinical protocols and best practices
- Participate in case conferences to monitor member progress and provide updates on targeted support plans
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license; New York, Illinois, and/or Michigan licensure preferred
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice for oncology patients
- Certification as a Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications preferred
- Fluency in Spanish, both written and verbal, is preferred
