Job Summary
To support a growing practice, the full-time salaried New York Licensed Palliative Care Nurse Navigator will engage with members experiencing serious illness, conducting comprehensive symptom assessments and facilitating care discussions through remote communication.
Key responsibilities
- Conduct comprehensive symptom assessments and support members with serious illness longitudinally
- Facilitate end-of-life care discussions and coordinate care with healthcare teams
- Participate in case conferences and collaborate with non-clinical team members to address social determinants of health
Required qualifications
- Active, unrestricted Registered Nurse (RN) license, with New York licensure preferred
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice for oncology patients
- Preferred certifications include Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications
- Fluency in Spanish, both written and verbal, is preferred
