Job Summary
To support individuals with serious illnesses, the full-time New York Licensed Palliative Nurse Navigator will engage in comprehensive assessments and care coordination through remote communication, focusing on goals, advance care planning, and symptom management while working flexible hours, including occasional Saturdays.
Key responsibilities
- Conduct comprehensive symptom assessments and facilitate end-of-life care discussions with members and their care networks
- Manage clinical escalations and provide support for members with serious illnesses, ensuring adherence to care protocols
- Collaborate with healthcare teams and participate in case conferences to monitor member progress and optimize care delivery
Required qualifications
- Active, unrestricted compact Registered Nurse (RN) license; New York, Illinois, and/or Michigan licensure preferred
- Degree in Nursing (ADN or BSN) required; BSN preferred
- At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice for oncology patients
- Certification as a Certified Hospice and Palliative Nurse (CHPN) or oncology-related certifications preferred
- Fluency in Spanish, both written and verbal, is preferred
