Job Summary

A company is looking for a Patient Care Navigator (REMOTE).

Key Responsibilities

• Collaborates with physicians to identify and admit patients for home treatment
• Acts as the main point of contact and care coordinator for patients post-inpatient discharge
• Assists in coordinating patient needs with community resources and financial assistance

Required Qualifications

• Bachelor of Social Work
• Two years of experience in a social work role with knowledge of patient service coordination
• Master of Social Work is preferred
• Care Coordination and Transition Management certification is preferred
• Knowledge of the geographical area of the assigned market is preferred
