Job Summary

A company is looking for a Pediatric Pharmacist (Remote) - NY Licensure.

Key Responsibilities

• Conduct thorough medication reviews and collaborate with healthcare providers to develop management plans for pediatric patients
• Provide remote consultations to caregivers and educate patients on medication use and adherence
• Maintain accurate documentation of medication management interventions and participate in quality improvement initiatives

Qualifications

• Doctor of Pharmacy (PharmD) from an accredited program; active pharmacy license required
• Board certification in Pediatric Pharmacy (BCPPS) preferred
• Minimum 4 years of pediatric pharmacy experience with strong clinical knowledge
• Proficient in telehealth platforms and EHR systems
• Proven ability to work independently and manage caseloads effectively
