New York Licensed Pharmacy Coordinator

Job Summary

Supporting centralized pharmacy operations in a remote environment, the full-time New York Licensed Pharmacy Coordinator will manage medication reconciliation, patient outreach, and medication access coordination across multiple hospitals and ambulatory care settings.

Key responsibilities

• Conduct medication reconciliation through patient interviews and external pharmacy records, documenting histories for pharmacist review
• Assist with medication coordination and logistics for Hospital-at-Home patients, ensuring medication availability and delivery
• Facilitate prior authorization processes and coordinate with insurance providers and pharmacies to ensure medication access for patients

Required qualifications

• High school diploma or equivalent required
• Experience supporting transitions of care programs
• Experience with Epic EHR
• Experience with prior authorization workflows
• Experience in telepharmacy or remote pharmacy services
