Job Summary

A company is looking for a New York Licensed Physical Therapist (Care Navigator) to provide remote support for patients' rehabilitation.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy programs
• Direct and assist users in therapeutic exercises and functional training through a technology platform
• Maintain records and provide feedback on patient progress to the evaluating clinician

Required Qualifications

• Active New York PT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy and comfortable using multiple platforms (EHR, messaging, scheduling)
• Experience in behavioral modification or Health Coach Certification is a plus
