Job Summary

A company is looking for a New York Licensed Physical Therapist (Care Navigator) - REMOTE.

Key Responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through a technology platform
• Maintain necessary records and provide ongoing feedback to evaluating clinicians on patient progress

Required Qualifications, Training, and Education

• Active New York PT license
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy, with the ability to navigate multiple platforms effectively
• Experience in behavioral modification or Health Coach Certification is a plus
