Job Summary

To support patient success remotely, the full-time New York Licensed Physical Therapist (Care Navigator) will provide remote monitoring services, assist users in therapeutic exercises, and maintain records while fostering relationships to enhance musculoskeletal health.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy programs
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through the technology platform
• Maintain necessary records and provide ongoing feedback to the evaluating clinician on patient progress

Required qualifications

• Active Physical Therapy license in New York
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms efficiently
• Experience in behavioral modification or Health Coach Certification is a plus
