Job Summary

As a Care Navigator, the full-time New York Licensed Physical Therapist will provide remote monitoring services, assist patients with therapeutic exercises, and maintain communication with evaluating clinicians to support patient recovery and musculoskeletal health.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy programs
• Direct and assist users in therapeutic exercises and functional training through a technology platform
• Maintain necessary records and provide ongoing feedback on patient progress to evaluating clinicians

Required qualifications

• Active PT license in New York
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms efficiently
• Experience in behavioral modification and Health Coach Certification is a plus
