Job Summary

As a Care Navigator, the full-time remote New York Licensed Physical Therapist will provide remote monitoring services, assist patients with therapeutic exercises, and maintain records while supporting musculoskeletal health and fostering patient relationships.

Key responsibilities

• Provide remote monitoring services to support patients in their home-exercise therapy program
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through a technology platform
• Maintain necessary records and feedback to clinicians regarding patient progress outside of the clinic

Required qualifications

• Active Physical Therapist license in New York
• Graduated from an accredited physical therapy program
• 2+ years of experience in musculoskeletal health is a plus
• Tech-savvy with the ability to navigate multiple platforms efficiently
• Experience in behavioral modification; Health Coach Certification is a plus
