Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with NY Licensure for a remote day shift position.

Key Responsibilities:

• Manage patient census to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources for patient care
• Provide patient education on chronic condition management and perform medication management

Required Qualifications:

• Active New York License (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote, HIPAA-compliant environment
• Experience with chronic care management
