Job Summary

New York Licensed Practical Nurse Coordinator is a full-time remote position responsible for managing patient care coordination, communication with healthcare providers, and ensuring high-quality care services.

Key Responsibilities

• Complete individualized patient care plans and perform care management services
• Maintain frequent contact with patients for care coordination and compliance management
• Document all client communications accurately and keep the Team Supervisor informed of care plan issues

Required Qualifications

• Must be a Licensed Practical Nurse (LPN)
• Comfortable with extensive phone communication
• Compassionate and empathetic towards patients
• Ability to multitask and remain organized in a fast-paced environment
• Proficient in using a computer and Microsoft Office programs
