New York Licensed Primary Care Physician

Job Summary

To expand access to primary care, the full-time New York Licensed Primary Care Physician will deliver remote, video-based comprehensive clinical care, including preventive care, sick visits, and chronic condition management, while managing a dedicated panel of patients.

Key responsibilities:

• Provide high-quality preventative, acute, and chronic care in a fully-remote, virtual setting
• Elevate remote care by offering compassionate and meaningful patient encounters
• Manage patient referrals to specialists or acute care services when clinically indicated

Required qualifications:

• Active New York medical license
• Board-Certified in Family or Internal Medicine
• 3+ years of post-residency work experience
• A minimum of 2+ years experience in an in-person primary care setting
• Experience with virtual visits and prior experience with EPIC EMR
