New York Licensed Primary Care Physician

Job Summary

To expand access to primary care, the full-time New York Licensed Primary Care Physician will deliver comprehensive remote clinical care, including preventive services, sick visits, and chronic condition management, while managing a dedicated patient panel and collaborating with a supportive team.

Key responsibilities

• Provide high-quality preventative, acute, and chronic care in a fully remote setting
• Manage a dedicated panel of patients and refer them to specialists as needed
• Engage in population health initiatives to improve patient outcomes and reduce hospital readmissions

Required qualifications

• Board-Certified in Family or Internal Medicine
• Active New York medical license
• 3+ years of post-residency work experience, with at least 2 years in an in-person primary care setting
• Experience with virtual visits and familiarity with EPIC EMR
• Willingness to commit to 40 hours per week, including 3x12-hour shifts on weekends and one additional weekday
