Job Summary

To support a growing Medicaid and PACE program, the remote New York Licensed Registered Dietitian will conduct nutrition assessments, provide individualized counseling, and manage referrals for Medicaid beneficiaries while collaborating with internal teams to enhance patient care.

Key Responsibilities

• Conduct nutrition assessments for Medicaid beneficiaries referred for counseling and meal services
• Manage and follow up on existing Medicaid referrals, ensuring timely engagement and documentation
• Provide individualized nutrition counseling and care planning tailored to patient needs and goals

Required Qualifications

• Licensed Registered Dietitian (RD) credential
• Credentialed and authorized to practice in New York State
• Up to date on all continuing education requirements per New York State regulations
• Strong clinical judgment and patient counseling skills
• Ability to manage caseloads independently in a fast-paced environment
