Job Summary

A company is looking for a Registered Nurse (RN) for a remote care management opportunity.

Key Responsibilities:

• Assist patients by providing information, coordinating services, and offering support remotely
• Conduct outreach to patients for follow-ups, wellness checks, and care coordination
• Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems

Qualifications:

• Registered Nurse (RN) license in New York State
• 2-3 years of experience in a medical office, preferably in chronic care management or telehealth
• Proficiency in EMR systems and medical terminology
• Bilingual in Spanish highly preferred
• Experience working in a remote healthcare setting is a plus
