Job Summary
To support a growing healthcare initiative, the full-time Remote Care Manager RN-New York Licensed will engage with members telephonically, develop individualized care plans, and coordinate care while working remotely.
Key responsibilities
- Engage with members and coordinate their care, dedicating 50-75% of the day to telephonic communication
- Develop individualized care plans by compiling clinical information and collaborating with members
- Document care management activities in electronic health records and ensure compliance with regulatory requirements
Required qualifications
- Active and unrestricted Registered Nurse (RN) license in the state of New York
- Associate's Degree in Nursing or related field
- Minimum of 3 years of nursing experience
- At least 2 years of case management, discharge planning, or home healthcare coordination experience
- Experience working with individuals with Social Determinants of Health (SDoH) needs or chronic medical conditions
