Job Summary
To support a growing healthcare initiative, the full-time Remote Care Manager RN-New York Licensed will engage with members telephonically, coordinate care plans, and ensure comprehensive support for dual eligible populations while working remotely.
Key responsibilities
- Engage with members to develop individualized care plans and provide disease management education
- Coordinate care with interdisciplinary teams to address health and social needs
- Document care management activities meticulously in electronic health records and meet regulatory requirements
Required qualifications
- Active and unrestricted Registered Nurse (RN) license in New York
- Associate's Degree in Nursing
- Minimum of 3 years of nursing experience
- At least 2 years of experience in case management or home healthcare coordination
- Experience working with individuals with Social Determinants of Health (SDoH) needs
