Job Summary

A company is looking for a Registered Nurse - Care Manager.

Key Responsibilities

• Provide patient-directed and disease-specific self-management education and psychosocial support
• Monitor and measure patient progress, ensuring timely and complete documentation
• Collaborate with the care team to assess, facilitate, plan, and advocate for appropriate services

Required Qualifications

• BSN (preferred) or Associate's Degree in Nursing (ASN) with a Bachelor's degree in another discipline
• Five (5) years clinical experience in care management, internal medicine, acute care, or home-based experience
• Ability to navigate complex medical and social systems
• A collaborative and flexible spirit
• Strong computer skills, including proficiency in Microsoft Office and electronic medical records
