New York Licensed RN Care Manager

Job Summary

To support a diverse patient population, the full-time New York Licensed RN Care Manager will manage care planning, coordination, and health assessments while working in a hybrid environment and residing within the New York Tri-State Area.

Key responsibilities

• Manage care planning and coordination for patients, ensuring effective health assessments and monitoring of service quality
• Oversee transitions for patients, facilitating smooth discharge planning from hospitals or nursing homes
• Identify health, environmental, and psychosocial risks, recommending and implementing interventions in collaboration with the Interdisciplinary Care team

Required qualifications

• Valid New York State RN License
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
• Associate's degree or higher in a relevant field
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
• Ability to effectively navigate and utilize various software and tools relevant to care management
