New York Licensed RN Care Manager

Job Summary

To support a diverse patient population, the full-time salaried New York Licensed RN Care Manager will manage care planning, coordination, and health assessments while working in a hybrid environment within the New York Tri-State Area.

Key responsibilities

• Manage care planning and coordination, ensuring quality and effectiveness of services
• Oversee patient transitions and facilitate discharge planning from healthcare facilities
• Identify health, environmental, and psychosocial risks to recommend and implement interventions

Required qualifications

• Valid New York State RN License
• Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings
• Associate's degree or higher in a related field
• Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole
• Ability to effectively navigate and utilize various software and tools relevant to care management
