Job Summary

A company is looking for a Remote RN Case Manager.

Key Responsibilities

• Provide support to dual eligible populations (Medicare and Medicaid members)
• Develop proactive care plans to address health outcomes and ensure overall wellness
• Conduct assessments and collaborate with interdisciplinary team members to address members' needs

Required Qualifications

• RN License required
• 3-5 years of clinical experience required
• 2-3 years of case management, discharge planning, and/or home health coordination experience
• Bilingual candidates preferred
