New York Licensed RN Case Manager

Job Summary

Working remotely from anywhere in the U.S., the full-time New York Licensed RN Case Manager will coordinate referrals for high-risk members, provide member education on illnesses and treatments, and support case management and quality improvement programs.

Key responsibilities:

• Collaborate with physicians and multidisciplinary teams to develop and maintain coordinated care plans for members
• Assist members and families in obtaining referrals to specialists while providing tailored counseling and support
• Partner with physicians to create and maintain individualized Member Care Plans and facilitate communication among care team members

Required qualifications:

• Bachelor of Science in Nursing (BSN) or 5+ years of case management experience in lieu of a BSN
• Unrestricted current RN licensure in the state of New York
• 3+ years of diverse clinical experience as a Registered Nurse, preferably with acutely ill members
• 2+ years of experience in health plan case management, complex and disease case management
• Experience in a remote and telephonic role
