New York Licensed RN Case Manager
Job Summary
To support a growing practice, the full-time New York Licensed RN Case Manager will coordinate comprehensive patient care, engage in ongoing care management, and collaborate with multidisciplinary teams while working remotely.
Key responsibilities
- Conduct initial intake calls and ongoing patient outreach to review care plans and monitor progress
- Provide care coordination for a panel of complex patients, ensuring needs are met and interventions are implemented
- Participate in interdisciplinary team meetings and quality improvement processes to enhance patient care delivery
Required qualifications
- Current RN License in good standing in New York or an unencumbered multistate license
- Bachelor's degree in nursing required, or an associate's degree in nursing with another bachelor's degree
- Minimum of 4 years of clinical experience, including at least 2 years in case management within home health care or similar settings
- Certified Case Manager (CCM) certification or commitment to obtain when eligible
- Geriatric care experience is highly desired
