New York Licensed RN Case Manager

Job Summary

To support a growing healthcare team, the remote New York Licensed RN Case Manager will coordinate referrals for high-risk members, provide education on illnesses and treatments, and manage care plans while working full-time in a collaborative environment.

Key responsibilities:

• Member Care Coordination - Collaborates with physicians and teams to develop and maintain coordinated care plans
• Member Referral Support - Assists in obtaining referrals and provides tailored counseling to members and families
• Care Plan Development - Partners with physicians to create and maintain individualized Member Care Plans

Required qualifications:

• Bachelor of Science in Nursing (BSN) or 5+ years case management experience in lieu of BSN
• Unrestricted current RN licensure in the state of New York
• 3+ years of diverse clinical experience as a Registered Nurse, preferably with acutely ill members
• 2+ years of experience in health plan case management, complex and disease case management
• Experience in a remote and telephonic role
