New York Licensed RN Case Manager

Job Summary

To support high-risk members, the remote New York Licensed RN Case Manager will coordinate referrals, educate members on their health conditions, and collaborate with healthcare teams to develop and manage individualized care plans.

Key responsibilities:

• Collaborate with physicians and multidisciplinary teams to develop and maintain coordinated care plans for members
• Assist members and families in obtaining referrals to specialists while providing tailored counseling and support
• Facilitate communication among care team members to ensure effective alignment of care plans and address member needs

Required qualifications:

• Bachelor of Science in Nursing (BSN) or 5+ years of case management experience in lieu of a BSN
• Unrestricted current RN licensure in the state of New York
• 3+ years of diverse clinical experience as a Registered Nurse, preferably with acutely ill members
• 2+ years of experience in health plan case management, including complex and disease case management
• Experience in a remote and telephonic role
