New York Licensed RN Case Manager

Job Summary

To support a growing healthcare practice, the full-time Remote RN Case Manager - Medicare & Medicaid will manage telephonic engagement and care coordination for members with complex health needs, ensuring compliance with regulatory requirements while working remotely.

Key responsibilities

• Conduct telephonic engagement with members to develop individualized care plans and provide evidence-based disease management education
• Collaborate with interdisciplinary care teams to address barriers to care and ensure seamless healthcare experiences for members
• Document care management activities meticulously in electronic health records and meet performance metrics for member engagement

Required qualifications

• Active and unrestricted Registered Nurse (RN) license in the state of New York
• Associate's Degree in Nursing (ASN)
• Minimum 3+ years of nursing experience
• Minimum 2+ years of experience in case management, discharge planning, or home healthcare coordination
• Experience providing care management for Medicare and/or Medicaid members
