New York Licensed Senior Care Manager

Job Summary

A company is looking for a Senior Care Manager (RN) to assess, plan, and implement complex care management activities for members.

Key Responsibilities

• Develop and continuously assess care plans/service plans, collaborating with providers to address member needs
• Coordinate care between members, families, and care provider teams to ensure person-centered services
• Monitor and revise care plans as necessary, identifying barriers and improving care quality in compliance with regulations

Required Qualifications

• Active New York State Registered Nurse License
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience in care management
• Experience in developing and implementing care plans
• Knowledge of state and federal regulations related to healthcare services
