Job Summary
To support quality healthcare outcomes, the full-time New York Licensed Senior Care Manager will assess, plan, and implement complex care management activities while working remotely and collaborating with members, families, and healthcare providers.
Key responsibilities
- Develop and continuously assess personalized care plans, collaborating with providers to address members' unmet needs
- Coordinate care between members, families, and care provider teams to ensure appropriate person-centered services
- Monitor care plans and member progress, revising plans as necessary to meet goals and improve care quality
Required qualifications
- New York RN State Licensure required
- Degree from an accredited school of nursing or a Bachelor's degree in Nursing
- 4 to 6 years of related experience in care management
- Experience with telephonic and digital outreach to assess member needs
- Knowledge of state and federal regulations related to healthcare services
