New York Licensed Social Care Navigator

Job Summary

Supporting New York's Medicaid Section 1115 Waiver program, the remote Bi-Lingual Spanish/English Social Care Navigator will manage member communications, assess eligibility for services, and coordinate referrals while ensuring compliance with Medicaid and HIPAA regulations.

Key Responsibilities

• Manage inbound and outbound member communications via phone and email using various technology platforms
• Conduct outreach to screen potential members and navigate them to appropriate service providers based on health-related social needs
• Track and follow up on referrals to ensure service delivery and maintain accurate documentation in internal systems

Required Qualifications

• High school diploma (Associate degree preferred)
• 3-5 years of experience in customer service, call centers, social services, or care navigation
• Experience in a virtual contact center environment (Five9 preferred)
• Strong communication and member engagement skills
• Ability to manage high-volume interactions in a compliance-driven setting
